BayouSide's signature membership is $129 per month, with household, student, dependent, and employer options. Every plan pairs unlimited included primary care with priority same-day scheduling and one-on-one provider attention. Enrollment is completed in office.

Everyday medical care

Primary care, urgent walk-ins & sick visits

Preventive care, physicals, EKG services, new concerns, ongoing condition management, medication review, screenings, and coordinated follow-up for patients ages 13+.

New-patient primary care visit

$150–$250

Estimated self-pay range for the office visit. Labs, medications, and procedures are separate unless confirmed otherwise.

Established-patient or sick visit

$100–$150

For non-emergency illness, a new symptom, or routine follow-up. Complexity may affect the final charge.

Urgent care walk-in visit

Estimate provided before care

For non-emergency, time-sensitive needs during office hours. Availability, testing, medications, and procedures can affect the final charge.

Annual preventive physical

$150–$225

Estimated self-pay range. Outside laboratory work and additional problem-focused care may be billed separately.

Sports, school, camp, employment or other physical

Call for current fee

Bring every required form. Additional labs, drug testing, EKG, or specialized certification requirements may be separate and are subject to availability.

EKG (electrocardiogram)

Call for current fee

Available when clinically indicated or required for an eligible physical. The visit, interpretation, and any additional testing or referral may be billed separately.

Women’s wellness or Pap visit

$175–$250

Estimated self-pay range. Laboratory, pathology, and additional testing are not included unless specifically quoted.

Men’s & women’s health

Hormone evaluations & ongoing care

Symptom review, medical history, appropriate laboratory testing, a personalized care discussion, and continued monitoring when treatment is appropriate.

Initial hormone evaluation

$150–$250

Estimated self-pay range for the clinical visit. Required laboratory testing and prescriptions may be separate.

Hormone follow-up or lab review

$100–$175

Estimated self-pay range for monitoring, results review, and treatment-plan adjustments.

Ongoing TRT or HRT program

$130–$200/month

Local-market planning range when care is structured as a self-pay program. Confirm whether visits, medication, supplies, and labs are included.

Hormone pellet therapy

The final quote should identify whether the pellet medication, procedure, supplies, follow-up, and required laboratory testing are included.
