You do not need to diagnose yourself. Choose the closest match, then the team will direct you to the correct intake packet.
For new-patient visits, annual wellness, sports, school or employment physicals, EKG services, chronic condition follow-up, and medication management.
Helpful to have ready
Insurance card, photo ID, medication list, allergies, specialist information, and available prior records.
For non-emergency illness, minor injuries, infections, rashes, or other time-sensitive concerns during office hours.

Photo ID, insurance card if using insurance, medication and allergy list, symptom start date, temperature if known, and any recent test results.
For men's TRT evaluations, women's hormone concerns, perimenopause, menopause, hormone pellet therapy, or ongoing hormone care.

Symptom timeline, current hormones or supplements, prior lab results, relevant screening history, and fertility goals when applicable.
For routine wellness labs or provider-selected hormone, thyroid, metabolic, nutrient, inflammation, heavy metal, environmental toxin, mold, and related panels.

Your concerns and exposure history, current medications and supplements, prior lab results, relevant diagnoses, and any instructions about fasting or specimen timing.
